Weekly Wellness Concierge Checks
This is a weekly service with a goal to keep your loved ones out of the hospital long term. I will keep an eye on your loved ones and update you weekly as needed. As a member, you will receive the following services:
- 1 hour weekly in home visit/wellness check
- Vitals: Blood Pressure, Temperature, Heart Rate, Oxygen Level
- Heart, lung, circulation, mentation assessments
- Check home enviornment for safety
- Review any medical reports, upcoming visits and organize them
- Medication education, set-up and organize medications, make a schedule if needed, and review
- Review any medical tests
- Education on health issues
- Schedule appointments if needed
- Communicate with medical team if needed
- Weekly updates to family as needed
